PRP or mesotherapy for hair: differences and clinical selection

PRP/PRF and scalp mesotherapy both use multiple small injections, so they are often treated as interchangeable. They differ in material, preparation and role in a treatment plan. Neither should be prescribed from a photograph or a vague statement that hair “feels thinner”.

What PRP and PRF mean
PRP is prepared by drawing venous blood and centrifuging it to obtain plasma with a higher platelet concentration. PRF uses a different protocol and a fibrin matrix. The resulting material depends on the tube system, centrifugation and injection technique. It is autologous rather than a vitamin cocktail, but that does not make it universal or suitable for every medical condition.
What scalp mesotherapy means
Mesotherapy introduces a manufactured product in small amounts into the scalp. Formulas may contain amino acids, peptides, hyaluronic acid or other components. The procedure name does not reveal what will be injected. Patients should know the product name, regulatory status and reason for choosing that formula.

When PRP/PRF may be considered
PRP is commonly included in a combined plan for androgenetic thinning and selected non-scarring conditions when follicles remain viable. It may complement evidence-based baseline therapy rather than replace it. Active infection, important blood abnormalities, certain medication or unstable health can postpone or exclude treatment.
When mesotherapy may be considered
The decision depends on ingredients and the clinical target, not a fashionable cocktail name. It may be used as an adjunct for selected scalp or follicular concerns. If shedding is driven by iron deficiency, thyroid disease, acute telogen effluvium or scarring alopecia, a local cocktail alone cannot solve it.
Diagnosis comes first
Similar-looking thinning may be androgenetic, telogen, alopecia areata or scarring disease. Examination considers distribution, variation in fibre diameter, follicular openings, scale and inflammation. Testing is selected only where the history supports it.

Treatment course and assessment
There is no responsible universal schedule of “four sessions for everyone”. Number and interval depend on diagnosis, method and response. Temporary tenderness, pinpoint marks, small bruises or headache may occur. Improvement is not judged the next morning: hair cycles slowly, so standardised photographs and repeat trichoscopy are more useful.
Ask about the diagnosis, treatment objective, system or product name, alternatives and outcome criteria. Be cautious when a clinic promises guaranteed new hair, skips scalp examination or gives every patient the same cocktail.

Sensation depends on injection technique, formula, area and individual sensitivity. Both involve multiple superficial injections.
Can the methods be combined?
A clinician may sometimes sequence treatments, but mixing products or doing everything in one visit requires a clear rationale.
When can I wash my hair?
Follow the clinician’s interval. Irritating products, hot water, sauna and intensive exercise are generally avoided immediately afterwards.
Can PRP regrow a completely smooth bald area?
If viable follicles are absent, plasma cannot create new ones. The cause and remaining follicular structures must be assessed.
Does mesotherapy replace oral vitamins?
No. A systemic deficiency is corrected according to testing and cause; local injections do not replace nutrition or medical treatment.
When should results be assessed?
Changes in shedding and growth are assessed over months rather than days. Consistent photographs and trichoscopy provide better comparison.
